Our Vision Statement

Three Rivers Community Health Group strives to provide accessible, patient centered primary and preventative health care including oral and behavioral health to the people of Perry, Hickman, and surrounding counties with a focus on continued quality improvement. We will continue a mission for excellence in meeting the current and future health care needs of our patients in a manner that is financially sound, meets cultural and community expectations and provides service at the highest professional level. Our vision includes the recruitment and retention of a qualified, caring staff who are culturally sensitive to individual patient and community needs; maintaining the appropriate level of medical technology; to provide in-service education and training programs to staff and to encourage opportunities for personal and professional growth. We desire to create healthier communities by being the premier model for the delivery of quality primary health care through the planning, development and strengthening of community-based health services. Our vision will be achieved by remaining consistent with our core values of service excellence, teamwork, leadership, and integrity.
