Understanding Coordinated Care

The best way to maximize the talents of the medical team members is to work as a physician-led medical team. Depending on the particular medical needs of the patient, skills combined in the team can consist of nurses, physicians, physician assistants, social workers, pharmacists, case managers, and other healthcare professionals. Members share information and participate in decision making based on their particular skills, all with the best care possible for the patient as the goal.

A message from our CEO
Perry County Medical Center has a long and proud history of providing excellent medical care in Perry County, Tennessee. Founded in 1979, the Center has been helping to serve the healthcare needs of the community for over 40 years now. The group incorporated as a not-for-profit and applied for a Rural Health Initiative Grant from the Federal Government and received funding to establish Perry County Medical Center. Shortly thereafter, the county funded a new 7,200 square foot building, and throughout the years it has continued to grow and provide quality health services to anyone seeking medical care at our facility regardless of their ability to pay for these services.
